That’s not a good idea. I agree that insurance companies would use it against the patient. At the most, I would allow it to be done at the discretion of the patient as long as they are made aware of all possible ramifications and contingencies. So do it only if the patient gives permission. As it is, patients can choose to make their doctors aware of things that they would rather keep off the record. This seems to work ok. Perhaps this is the best practice.
