The National Healthcare Decisions Day (NHDD) Initiative is a collaborative effort of national, state and community organizations committed to ensuring that all adults with decision-making capacity in the United States have the infomation and opportunity to communicate and document their healthcare decisions.

1. All adult Americans should have access to good information to make informed decisions about their healthcare now and in the future.

2. Less than 50 percent of the severely or terminally ill patients studied had an advance directive in their medical record.

3. Between 65 and 76 percent of physicians whose patients had an advance directive were not aware that it existed.

4. An overwhelming majority of the public supports laws giving patients the right to decide whether they want to be kept alive through medical treatments
