C/o Appeal Docket  Administrator Heather Funk
5 North 5th StreetHarrisburg, Pa 1710
Subject:Appeal and Request for Administrative Hearing June 20, 2013 Teresa A. Rollins File No 88573#
I am filing an administrative appeal. This response is in response to the section of the appeal titled Part D, E and F Part D: Facts which are being submitted that support my reasons for are as follows:I am 60 years old and recently retired from the teaching profession. I currently live homeless in Pennsylvania. In
2012, I attended a seminar on the topic of getting access to funds in
my retirement account, which I very desperately need for current living
expenses. At the time of the seminar, I was homeless and suffering from
duress because of my unemployment and lack of money.  I initially agreed
to a retirement payout of $1,000 per month, and later understood that I
could modify the terms of the payout as specified in a letter that I
was told was mailed to me at a temporary address. I
never received this communication from you, and I was subsequently
informed that the grace period for changing the terms of my payout has
expired.
The
$1,000 monthly retirement payout is not sufficient for me to securing
permanent housing and pay for my current medical expenses, as I being a
Diabetic have a life threatening condition and currently I am without
the ability to afford adequate medical care that the  pension coverage
once changed  will afford me. Portion E: Statement of Relief, what I want the Board to do:
Permit me to review the change options and allow me to choose the
options that best fit my circumstances. I would like to have a period to
review my options with a professional, as I need to understand fully
what my options are, and I never received the change notice to make
changes to cover my expenses.  I would like to make an informed decision
which would allow me to cover all of my expenses.Portion F: Why should the board Grant my Request:  Out
of all fairness, I never received the change notice; I would like to
exercise my right to review and to choose a different option as the $1,000 monthly retirement payout
is not sufficient for me to securing permanent housing and pay for my
current medical expenses, as I being a Diabetic have a life threatening
condition.
Currently I am without the ability to afford adequate medical care, as well as affordable housing. The
pension coverage once changed and my being granted a different payment
option will enable me to secure a permanent place to live, and adequate
medical care, as without an option change, I am facing a situation of
having no affordable place to live and inability to have adequate
medical diabetic care.  Thank
you for your consideration.  I may be reached at a neighbors address of
92 Daffodil Lane Levittown, Pa 19055
