To provide access to quality diabetes education and support to those living with diabetes in underserved communities in North Carolina. So WHERE you live doesn't determine HOW you live with diabetes

We have improved the health outcomes of people living with diabetes in 167 hometowns and 26 counties of North Carolina. We are strategically moving our reach to include all of North Carolina and to build a model that is usable nationwide.

1. Everyone with diabetes should have access to quality diabetes education.

2. WHERE you live should not determine HOW you live with diabetes.

3. Hospital diabetes education programs are closing due to lack of revenue- DMS replacing those programs at no cost to participants or the hospital

4. We receive NO state or federal funding
